Regular activity is healthy and fun and more and more people are becoming more active every day. The important thing is to make sure we are safe when performing physical activity. Most physical activity is safe for the majority of people, but some should check with their doctor before starting to implement more physical activity.
If you planning on becoming more physically active, please answer the questions below. If you are between 15 and 69 years of age, this will tell you if you should check with you doctor before starting. If you are over 69 years of age, please check with your doctor.

If you answered YES to one or more questions:
Talk with your doctor BEFORE becoming more physically active. Tell your doctor about this questionnaire and which questions you answered YES to. Discuss which exercises are safe for you and which to avoid. It may be that you need to start slowly and gradually increase. Talk to you doctor about which kinds of activities you wish to participate in and follow his advice.
If you answered NO to all of the above questions:
It is advised to start slowly for safety and gradually increase. If anything changes and you answer YES to the above questions, tell your trainer/coach immediately. You will want to discuss with a doctor about possibly changing your physical activity plan.
